Four-year-old female with abdominal pain (discussion and diagnosis)

At the tertiary hospital, an ultrasound (Fig. 1) detected an enlarged type 1 choledochal cyst with peripancreatic fluid, consistent with acute pancreatitis. The obstructing stone could not be visualised anymore, so medical management was continued, and the patient was subsequently discharged with a plan to carry out a definitive resection of the choledochal cyst in the near future. Two months after her recovery from acute pancreatitis, the patient underwent laparoscopic cholangiogram, open cholecystectomy, choledochal cyst excision and Roux-en-Y hepaticojejunostomy, which she tolerated well without any complication.
