During the past decade, there has been tremendous movement in endoscopy beyond the gastrointestinal lumen to the third space and beyond. Natural Orifice Translumenal Endoscopic Surgery, in its original form, may have entered and exited the surgical endoscopy space much like a tornado with a rapid accumulation of momentum and equally rapid quiescence, but left in its path a massive disruption of our boundaries for treating GI pathologies. It also served as a catalyst for game-changing innovation in techniques and devices, such as sub-mucosal endoscopy and defect closure methods and tools. Full thickness resection is an excellent example of the innovative process. As a concept, it marries evolution in technique with availability of technology for the purpose of filling unmet needs in areas of organ-sparing tissue resection, staging, and tissue acquisition.
