J. Heaton and F. Jockenhovel both speculate in this issue of the JMHG [1,2] that there is a suspicion that the importance of androgen status to male wellbeing is a modern, perhaps even commercial, construct. Furthermore that the reasons for the use of androgen substitution for late onset hypogonadism (LOH) maybe the notion that the clinical manifestations of LOH maybe physiological during ageing and androgen therapy is considered an option to improve lifestyle rather than quality of life. Both assumptions are wrong! As female and male hormone replacement therapy (HRT) has become a topic of increasing interest and debate throughout the world the time is more than right for a solid scientific discussion and to review all the evidence for HRT.
