On Mar. 23, 2005, the third-largest refinery in the U.S. experienced a release of a highly flammable gas. The gas was ignited by an idling pickup track, and the resulting explosion killed 15 people. An independent industry investigation board, headed by former Secretary of State James Baker, conducted an inquiry of the explosion and released its findings (the "Baker Panel Report") in January 2007.
