There is little doubt that safety in the UK's offshore sector has increased markedly since Piper Alpha was destroyed by fire in 1988 (see box, right). Nevertheless, Britain's Health and Safety Executive (HSE) has just completed a three-year investigation and inspection of nearly 100 offshore installations, which has concluded that, while significant improvements have been made, more must be done. That stark message came at the launch of the Key Programme 3 (KP3) report - a major investigation carried out by the HSE's Aberdeen-based Offshore Division into the safety and integrity of offshore installations and the equipment on them.
