NTSB identifies fatigue and watch schedule as factors in grounding of Alaska ferry

Failure by the master and chief mate of the Alaska Marine Highway System ferry LeConte to recognize that a course change chosen by the chief mate would place the ferry on the wrong side of a daymark led to the grounding of the vessel on Cozian Reef (PM #82), 30 miles north of Sitka, Alaska, the National Transportation Safety Board has concluded.
