Canadian authorities have determined that a "large breaking wave" caused a whale-watching boat to capsize in 2015 off Vancouver Island, British Columbia, killing six people. There were 24 passengers and three crew on the 65-foot Leviathan II when it rolled over at aboui 1500 on Oct. 25, 2015, in Clayo-quot Sound near Tofino, B.C. Crew could not send a distress message and 45 minutes passed before rescue agencies learned about the accident.
