Abstracts from the 12th Annual Care Continuum Alliance Forum and the Integrated Care Summit

This year's annual meeting of the Care Continuum Alliance offered a showcase of robust new research, innovative care strategies, and emerging opportunities that mirror positive changes afoot for the newly named organization and the stakeholders it represents. The more than 60 concurrent and general sessions at the October 13-15 Forum 10 and Integrated Care Summit, in Washington, DC, demonstrated the breadth and depth of approaches to care among Care Continuum Alliance members and the broader wellness and care management community.
