Improving policy and practice to promote equity and social justice - a qualitative comparative analysis building on key learnings from a twinning exchange between England and the US

Les interventions de promotion de la santé communautaire ciblant les populations marginalisées et axées sur les déterminants sociaux de la santé (DSS) dans le but de réduire les inégalités en santé et d'aborder les processus d'exclusion constituent une stratégie importante de prévention et de contrôle des maladies non transmissibles (MNT), et de promotion de la santé des groupes défavorisés et démunis. Cet article tire parti des principaux enseignements d'un échange d'apprentissages entre le groupe en Angleterre, Communautés pour la Santé ( Communities for Health ) et des communautés aux États-Unis, Approches Raciales et Ethniques de la Sante Communautaire (Racial and Ethnic Approaches to Community Health, REACH US), deux initiatives qui luttent contre les inégalités en santé. Cet échange a été mené par un partenariat entre les centres américains de prévention et de contrôle des maladies ( US Centers for Disease Control and Prevention, CDC), l'Union Internationale de Promotion de la Santé et d'Éducation pour la Santé, le Département Anglais de la Santé, Health Action Partnership International et Learning for Public Health West Midlands. Cet article présente également une analyse qualitative qui recueille des informations sur des interventions communautaires spécifiques impliquées dans cet échange, et identifie les enseignements qui ont pu en être tirés. Cet échange d'apprentissages ainsi que l'analyse qualitative fournissent des éléments de compréhension intéressants pour la recherche future, des domaines prioritaires dans le champ des politiques et des pratiques pour aborder les DSS, promouvoir et soutenir la justice sociale et l'équité à travers le monde. Cet article met en avant certaines leçons essentielles à propos de l'utilisation des données, d'interventions communautaires basées sur les atouts et de l'importance d'un bon leadership en temps de crise et d'adversité. S'ils sont complexes et longs à mettre en place, de tels programmes ont le potentiel d'offrir à d'autres pays, notamment à ceux du Sud, de nouvelles perceptions et perspectives qui contribueront à leur tour au domaine des DSS, et fourniront des stratégies et des actions concrètes afin de réduire efficacement les inégalités, et de promouvoir la santé de nos sociétés. Ces apprentissages essentiels ont le potentiel de contribuer à la communauté mondiale et à la documentation croissante sur les données probantes à propos des efforts efficaces dans la diminution des inégalités de santé.%Community health promotion interventions, targeted at marginalised populations and focusing on addressing the social determinants of health (SDH) to reduce health inequalities and addressing the processes of exclusion, are an important strategy to prevent and control non-communicable diseases (NCDs) and promote the health of underprivileged and under-resourced groups. This article builds on key lessons learnt from a learning exchange between Communities for Health in England and the Racial and Ethnic Approaches to Community Health across the US (REACH US) communities that ate tackling health inequities. It presents a qualitative analysis further capturing information about specific community interventions involved in the exchange and identifying lessons learnt. This exchange was led by a partnership between the US Centers for Disease Control and Prevention, the International Union for Health Promotion and Education, the Department of Health of England, Health Action Partnership International, and Learning for Public Health West Midlands. These efforts provide interesting insights for further research, priority areas of action for policy and practice to address the SDH and to promote and sustain equity and social justice globally. The article highlights some key lessons about the use of data, assets-based community interventions and the importance of good leadership in times of crisis and adversity. Whilst complex and time-consuming to arrange, such programmes have the potential to offer other countries including the global south new insights and perspectives that will in turn contribute to the SDH field and provide concrete strategies and actions that effectively reduce inequities and promote the health of our societies. The key learnings have the potential to contribute to the global community and growing documentation on evidence of effective efforts in the reduction of health inequities.
