On the evening of 10 October 2003, two boiler cleaning specialists were preparing to examine the starboard boiler on board the LNG carrier Hilli when a halogen lamp was placed inside the boiler steam drum. An explosion immediately followed, injuring both men, one fatally. The UK's Marine Accident Investigation Branch (MAIB) report into the incident has just been published and makes a number of important recommendations for marine engineers and others.
