During severe sepsis, organ dysfunction is an important predictor of poor outcomes. Animal and human studies suggest that macrophage migration inhibitory factor (MIF) plays a detrimental role in organ dysfunction in severe sepsis. However, MIF is a key mediator of the innate immune response and is necessary for resistance to certain infections. We provide a broad overview of the beneficial and detrimental effects of MIF in sepsis. The interplay between MIF and glucocorticoids, which may play a critical role in sepsis, is also examined.
