Concerns regarding the complex issue of increased turnover in child social welfare workers continue to alarm health care leaders in every sector of health care especially turnover amongst polio vaccination employees working under Union Council Medical Officer. This study aims at presenting a framework, developing concrete research hypotheses and hence test them to explain the variation in the relationship between bureaucracy, perceived threat to life and turnover intention. Four indicators of organizational bureaucracy have been considered; Formalization, Centralization, Restricted Decision Making and Stability Orientation. Polio eradication program in Pakistan has not been able to achieve its goal thus remains one of the three polio endemic countries in the world as of 2015. Therefore, this paper serves a critical purpose by intending to answer three most important yet overlooked question; What is the influence of dimensions of organizational bureaucracy on the turnover intention of polio vaccination employees and how does perceived threat to life mediate this relationship? We have selected a national sample of polio vaccination employees and hence ensured equal representation from all four provinces of Pakistan. Out of the four dimensions of bureaucracy, only stability orientation does not have an impact on turnover intention. Mediation analysis shows that formalization and centralization impact turnover intention, however, this impact is mediated by perceived threat to life. This paper will serve as a useful guideline for policy makers under Prime Minister's National Task Force working to interrupt poliovirus transmission and eventually achieve the goal of eradication.
