PENALTY VIOLATIONS

This issue's reporting period for penalties issued by the US D.O.T., Pipeline and Hazardous Materials Safety Administration covers May and June. Penalties in the form of Compromise Orders, Default Orders, Notices of Probable Violation, and Final Orders were issued to 45 companies. Also included in this issue's roundup are the Closeout Letters, Warnings, and Notices of Withdrawal issued by PHMSA. Violations of the HMR covered many areas of the hazardous materials regulations. Highlighted recurring issues are those violations committed by failing to provide both initial and recurrent hazardous materials employee training, as well as failing to develop and adhere to security plans. Following this summary is a chart of the penalties covered by this period, organized by company category, divided by violation type, and with reference IDs provided. The sum of all fines during this period amounted to over USD$340,000. Additional fines were proposed amounting to over USD$57,000, subject to appeal and/or compromise.
