Rolling back the years

The US National Transportation Safety Board (NTSB) has issued a suite of recommendations as a result of its investigation into the October 22, 2009 incident in Indianapolis, Indiana, where an AmeriGas Propane Inc tank truck rolled over on a ramp leading off 1-69 to 1-465. The rollover occurred when the driver over-compensated after drifting into the left-hand lane, occupied by a passenger car. As a result of the manoeuvre, the trailer rolled over and decoupled from the tractor, penetrated the guardrail and collided with the bridge footing, rupturing the cargo tank and allowing a vapour cloud of propane to escape. The truck driver and the driver of the car were both seriously hurt in the ensuing fire and a number of other drivers received minor injuries.
