Drug therapy for the treatment of chronic heart failure and concomitant diseases // Medikation in der Behandlung der chronischen Herzinsuffizienz und Begleiterkrankungen
This article discusses traditional tools and drugtherapy in the treatment of chronic heart failure. The study investigates the drugs that both directlyreduce the occurrence of new symptoms andprevent new concomitant diseases. The studyinvestigates modern drugs for the treatment ofconcomitant diseases, as well as for the diagnosisof new ones. The authors consider the treatmentof chronic heart failure with immune andother concomitant diseases using diabetes mellitusas an example. New data on the increasedfrequency of various types of mechanical myocardialdyssynchronicity (atrio-ventricular, interventricularand intra-ventricular) and their combinationsin patients with chronic heart failure ofischemic origin, associated with type 2 diabetesmellitus, has been obtained. A relationship betweenthe severity and type of chronic heartfailure (depending on the functional class andleft ventricular ejection fraction) and myocardialdyssynchronicity was established. It was shown that an increase in the glycosylatedhaemoglobin levels was associated withan increase in the parameters of electrical andintra-ventricular mechanical dyssynchronicity.Data on the effects of cardiac dyssynchronicityon the structural and functional myocardial parametersin patients with chronic heart failure ofischemic origin and type 2 diabetes mellitus wasfurther defined. The effects of myocardial dyssynchronicityon cardiac remodelling, occurrenceof inadequate models of left ventriculargeometry and diastolic function were defined.
