Aims To identify the impact of a community integrated respiratoryteam on early discharge after exacerbation of COPD, need forpulmonary rehabilitation and care provision in the community. Methods A rolling audit of all referrals to the team to identifycomorbidity, complexity of disease and need for pulmonaryrehabilitation in south-west Essex to support a new integratedservice with pulmonary rehabilitation, oxygen services, andin-reach into hospital. Audit of patients reviewed for earlydischarge on the acute medical unit.
