Shared psychotic disorder (SPD) is defined as delusional beliefs of a subject that are transmitted to one or more people, who have close relations.Content of shared delusions should be identical or similar, and other individuals should provide support each other’s delusions. This case presentation is about a young girl, who had sudden-onset of delusions and her mother, who developed same delusions one month later. Primary Case: A 21 year old female started to suspect everything approximately two months ago. She started to state that her meal poisoned and she would be killed. She was hospitalized. Cranial MRI was normal. PANSS Score was 118. Olanzapine 10 mg/d and diazepam 10 mg/d were started. Paliperidone 6 mg/d was added on day 4 of the treatment upon persistence of severe delusions. Diagnosis of delusional disorder was made and the patient was in partial remission with partial insight when she was discharged at Day 14 of the treatment. Maintenance treatment included olanzapine 10 mg/d and paliperidone 6 mg/d. PANSS score regressed to 48 points at Week 6 of the treatment and she achieved complete remission. Secondary Case: A 51 years old female patient. Just one month after her daughter suffered above mentioned disorder, she started to claim that they were entrapped, they are besieged and their foods are poisoned. PANSS score was 111. She was started treatment with olanzapine 10 mg/d and diazepam 10 mg/d. She was discharged in partial remission on Day 14. PANSS score regressed to 43 points at Week 6 during follow-up period and the patient achieved complete remission. Shared psychosis is a result of pathological relationship between two or more people, who have close relations and are socially isolated. It is believed that more case presentations are required to understand pathophysiology of this unknown disease.
