I am writing in response to Fox and Grogan’s review1 of my book, Health Divided: Public Health and Individual Medicine in the Making of the Modern American State.2 Fox and Grogan critique Health Divided through the lens of Fox’s concept of “hierarchical regionalism.” Although any reader of my book will recognize my respect for Fox’s work, hierarchical regionalism is only one means through which the trajectory of health policy might be understood.
