BiPAP无创呼吸机治疗AECOPD合并呼吸衰竭疗效观察

摘要

目的：观察BiPAP无创呼吸机治疗AECOPD并呼吸衰竭的临床疗效。方法选取60例AECOPD合并呼吸衰竭患者，随机分为BiPAP无创呼吸机治疗组与常规治疗对照组，分析比较两组患者在治疗后2h、24h、3d、7d的呼吸频率、心率、收缩压、舒张压、血气分析、肺功能等变化，并比较住院时间。结果两组患者呼吸频率、心率、血气分析、肺功能经治疗后均改善，且在各观测时间点，BiPAP无创呼吸机治疗组较对照组变化更显著（P<0.05）；BiPAP无创呼吸机治疗组患者住院时间明显短于对照组（P<0.01）；治疗前后收缩压、舒张压变化差异无统计学意义。结论 BiPAP无创呼吸机治疗AECOPD合并呼吸衰竭临床疗效确切。%Objective To investigate the clinical efficiency of BiPAP non-invansive respiratory ventilator in the treatmen of Aecopd with respiratory failure.Medthods 60 AECOPD patients with respiratory failure were randomly divided into the BiPAP non-invansive treatment group and the routine treatment control group.The results of respiratory rate,heart rate,systolic blood pressure,diastolic blood pressure,blood gas analysis and pulmonary funcion in the two groups were compared in two hours,twenty-four hours,three days and seven days after treatment.And the difference of hospital time between the two groups were also analyzed. ResultsThe situation of respiratory rate,heart rate,blood gas analysis,pulmonary funcion in the two groups were improved.At each observation time,the changes in the BiPAP non-invansive treatment group were more remarkbly than the changes in the control group.The difference between the two groups was statistically significant(P<0.05).The hospital time of the BiPAP non-invansive treatment group was shorter than the control group,and the difference between the two groups was notably statistically significant (P<0.01).No obvious changes of systolic blood pressure,diastolic blood pressure were obversed after the therapies.The difference between the two groups was not statistically significant(P>0.05).Conclusions BiPAP non-invansive respiratory ventilator has the exact clinical efficiency in the treatmen of AECOPD with respiratory failure.The technique is worthy applying and spreading clinially.
