糖前康胶囊对糖尿病前期胰岛功能及白介素18mRNA表达的影响

         

摘要

Objective:To observe the effects of TangQianKang capsule on the regulating effects of glycometabolism,lipid metabolism and pancreatic islet function as well as inflammatory factors in patients during prediabetes.Methods:The 171 patients at prediabetic state were randomly divided into 85 cases in the control group and 86 cases in the observation group.The control group was treated with metformin,one tablet (0.25 g) per time,twice a day while the observation group was treated with TangQianKang capsule,three capsules per time,and three times a day.The two groups were treated for 12 weeks.The body weight,waist circumference and body mass index (BMI) were determined after treatment;and the serum lipid levels,blood glucose (FBG,2hPG) on an empty stomach and in two hours after meal,fasting insulin (FINS),glycosylated hemoglobin (HbA1c),24-hour urinary microalbumin (UTA),insulin resistance index (HOMA-IR),beta cell of islet secretion function index (HOMA-IS) and insulin sensitivity index (ISI)were detected before and after treatment;In addition,serum interleukin (IL-18) mRNA expression and hypersensitive c-reactive protein (hs-CRP) level were measured before and after treatment.Results:The body weight,waist circumference and BMI of the two groups after treatment were lower than those before treatment,and there was no significant difference between the two groups(P>0.05).The levels ofFBG,2 hPG,HbA1c,FINS,24 hUTA and HOMA-IR in the observation group were lower than those in the control group (P<0.05)while the levels of HOMA-IS and ISI were higher than those of the control group (P<0.05).Moreover,the number of the individual indexes of blood lipids changed into normal in the observation group was more than those in the control group (P<0.05).In addition,the levels of IL-18mRNA and hs-CRP in the observation group were lower than those in the control group (P<0.05).Conclusion:TangQianKang capsule can regulate glucose metabolism,lipidmetabolism,improve the function of beta cell of islet and delay the progression to diabetes.Its mechanism may be related to the regulation of factors as IL-18 and hs-CRP.%目的:观察糖前康胶囊对糖尿病前期患者糖代谢、脂代谢调节作用及对胰岛功能、炎性因子的影响.方法:将171例糖尿病前期患者随机分为对照组85例、观察组86例.对照组服用二甲双胍,1片(0.25g)/次,2次/d.观察组服用糖前康胶囊,3粒/次,3次/d.2组疗程均为12周.测量患者治疗后体质量、腰围、体质指数(BMI);检测治疗前后血脂水平、空腹和餐后2小时血糖(FBG、2hPG)、空腹胰岛素(FINS)、糖化血红蛋白(HbA1c)、24小时尿微量白蛋白(UTA),胰岛素抵抗指数(HOMA-IR)、胰岛β细胞分泌功能指数(HOMA-IS)和胰岛素敏感指数(ISI);检测治疗前后患者血清白细胞介素(IL-18) mRNA表达和超敏C-反应蛋白(hs-CRP)水平.结果:治疗后2组体质量、腰围和BMI均较治疗前下降,组间比较差异无统计学意义(P>0.05),观察组FBG、2 hPG、HbA1c、FINS水平、24 hUTA及HOMA-IR低于对照组(P<0.05),HOMA-IS和ISI高于对照组(P<0.05);观察组血脂单项指标转为正常例数多于对照组(P<0.05);观察组IL-18mRNA和hs-CRP水平低于对照组(P<0.05).结论:糖前康胶囊可调节糖尿病前期患者的糖代谢、脂代谢,改善胰岛β细胞功能,延缓进展为糖尿病,其作用机制可能与调节IL-18及hs-CRP等因子有关.
