电子血压计在招飞体检中的应用价值

摘要

Objective To evaluate the value of adopting electronic sphygmomanometers for measuring blood pressure ( BP ) of pilot cadets candidates. Methods A total of 900 young students were recruited by using random sampling method from the students taking part in the 2015 annual navy medical selection of pilot cadets. The consistency of BP values measured by electronic sphyg-momanometers, and mercury sphygmomanometers were respectively compared in calm state and mo-tion state. Results The consistency of the measurement results recorded by two kinds of sphygmo-manometers was compared by using interclass correlation coefficient ( ICC) and Bland-Altman plots. The ICC between systolic BP values and the ICC between diastolic BP had consistency. The research found out the systolic BP difference [(0. 57+3. 21) mmHg] and diastolic BP difference [(0. 71+3. 62) mmHg] in calm state, systolic BP difference (0. 61+3. 59) mmHg and diastolic BP difference (0. 81+3. 65) mmHg in motion state were all within the scope of the range (x-±1. 96s) by Bland-Altman plots for scatter distribution. Conclusion There was good consistency between the BP va-lues measured by two kinds of sphygmomanometers, so upper-arm electronic BP monitor can substi-tute for mercury sphygmomanometers in medical selection of pilot cadets.%目的：探讨电子血压计在招飞体检血压判定中的应用价值。方法参加2015年度海军招飞全面检测的学生中随机选取900名,在平静状态和运动负荷试验中使用臂筒式电子血压计和水银血压计同时测量血压,对测量的血压值进行数据分析,选取组内相关系数( interclass correlation coefficient,ICC)和Bland-Alt-man图示法进行一致性分析。结果平静状态下2种血压计测量的收缩压ICC为0.973和舒张压ICC为0.965,运动负荷后2种血压计测量的收缩压ICC为0.987和舒张压ICC为0.953,均具有一致性。平静状态下2种血压计测量的收缩压差值(0.57±3.21)mmHg、舒张压差值(0.71±3.62)mmHg,运动负荷后2种血压计测量的收缩压差值(0.61±3.59)mmHg、舒张压差值(0.81±3.65)mmHg,Bland-Altman散点图提示绝大多数散点分布在((x-±1.96s)范围内。结论臂筒式电子血压计测量结果比较稳定,与水银血压计测量结果一致性好,可替代水银血压计用于招飞体检的血压判定。
