目的：研究前列腺增生合并Ⅲ型前列腺炎患者前列腺组织中炎性细胞分布情况，探讨ⅢB型前列腺炎诊断标准的合理性。方法：选择前列腺增生患者123例，入院后均行经直肠超声检查，观察前列腺内部回声及血流动力学改变。所有患者麻醉前行尿四杯检查，按NIH诊断标准筛选出ⅢA型前列腺炎患者30例，ⅢB型前列腺炎患者28例。切除的前列腺增生组织用Nickel的前列腺炎症程度和主要炎症类型的诊断标准进行观察，分析前列腺液中炎性细胞与前列腺组织病理切片中炎性细胞分布的关系，以及两种类型前列腺炎超声表现差异。结果：28例ⅢB型前列腺炎患者组织切片中，腺体以轻度炎症为主，腺周以中度炎症为主，基质以重度炎症为主；30例ⅢA 型前列腺炎患者组织中炎症表现与ⅢB型相似。结论：按照最新的慢性前列腺炎诊治标准，可能有部分ⅢB型前列腺炎患者被误诊、误治，应引起重视。%Objective:To study the distribution of inflammatory cells in typeⅢ prostatitis tissue ,in order to explore the diagnostic criteria for typeⅢB prostatitis .Methods :We selected 123 cases of benign prostatic hyper‐plasia patients .All patients accepted transrectal ultrasonography and observed internal echo and hemodynamic chan‐ges of the prostate .Before anesthesia ,patients accepted Meares‐Stamey test ,28 cases of type ⅢB prostatitis pa‐tients and 30 cases of type ⅢA prostatitis patients were found among them .After operation ,we observed the pros‐tatic tissues according to the prostatitis diagnostic criteria of Nickel inflammation standard .We studied the relation‐ship of inflammatory cells between the prostatic fluid and the prostate tissue biopsy of glands ,glands week and the matrix ,and the difference between two types of prostatitis ultrasonography was also studied .Results :In the 28 ca‐ses of type ⅢB patients ,there were mainly mild inflammation in glands ,moderate inflammation in glandular week and severe inflammation in matrix ;in other 30 cases of type ⅢA prostatitis patient’s tissue ,the distribution of in‐flammatory cells in glands ,glandular week and in the matrix were similar with the type ⅢB .Conclusion :Accord‐ing to the latest standard of diagnosis and treatment of chronic prostatitis ,some type ⅢB prostatitis patients are misdiagnosed and mistreated .We should pain more attention to these patients .
