目的 探讨原发灶HER2状态与胃癌临床病理特征的关系.方法 根据胃镜活检和手术标本病检的结果,选取经病理组织学确诊为胃腺癌患者的石蜡标本144例.依据临床诊疗指南对淋巴转移灶、胃镜活检的标本、HER2的状态进行判定,并对检查的结果进行对比分析.结果 胃镜活检的HER阳性表达率是23.61％,手术标本是13.89％,两者HER-2状态的一致率达86.11％;102例淋巴结转移患者内有30例是原发灶HER2阳性,其表达率是29.41％,转移灶HER2阳性25例,表达率是24.51％,一致性为75.49％,有25例不一致的患者;患者性别、年龄、肿瘤大小和肿瘤位置与HER2状态无关(P＞ 0.05),肿瘤组织学分级、淋巴结转移、Lauren分型、TNM分期和浸润深度与HER2状态有关(P ＜0.05).结论 在淋巴结转移灶中对HER2进行检测可以较好地预测胃癌原发灶HER2的状态;患者性别、年龄、肿瘤大小和肿瘤位置与手标本HER2状态没有关联,患者组织学分级、淋巴结转移、Lauren分型、TNM分期和浸润深度与HER2状态有关.%Objective To explore the relationship between HER2 status of primary tumor and clinicopathological features of gastric cancer. Methods According to the results of gastroscopy biopsy and surgical specimen examination, 144 paraffin specimens of patients with gastric adenocarcinoma confirmed by histopathology were selected. According to the clinical diagnosis and treatment guidelines, the specimens of lymphatic metastases, gastroscope biopsy, and HER2 were determined, and the results of the examination were compared and analyzed. Results The positive rate of HER positive expression was 23. 61％, the surgical specimen was 13. 89％, and the coincidence rate of HER-2 status was 86. 11％. Among the 102 patients with lymph node metastasis, 30 cases were positive for HER2, and the expression rate was 29. 41％, 25 cases of HER2 positive in metastases, the expression rate was 24. 51％, the agreement was 75. 49％, and there were 25 cases of inconsistent patients; gender, age, tumor size and tumor location were not related to HER2 status (P＞ 0. 05), tumor Histological grade, lymph node metastasis, Lauren classification, TNM stage and depth of invasion were related to HER2 status (P ＜ 0. 05). Conclusion Detection of HER2 in lymph node metastases can predict the status of HER2 in gastric cancer. The gender, age, tumor size and tumor location are not associated with HER2 status in patients. Histological grade, lymph node metastasis, Lauren score type, TNM stage, and depth of invasion of HER2 status.
