上海市某三级甲等医院高额住院费用分析

摘要

目的：分析上海市某医院高额住院费用的构成情况，探索其影响因素，从而提出有针对性的建议和措施。方法：筛选上海市某三级甲等综合性医院2014年住院费用大于10万的高额住院病例共811例。采用SPSS19．0软件进行统计学处理。结果：高额住院病例费用中药费和耗材费最高，影响因素为性别、医疗付费方式、病种、科室、住院天数和疗效。结论：应当降低药费和材料费、加强重点人群健康教育、监控重点疾病和科室、减少平均住院天数、提高医疗水平、完善临床路径、探索按病种付费制，从而有效控制高额住院病例。%Objective: Analysis on the composition of high hospitalization expenses and explores the influencing factors so as to put forward some specific measures and suggestions.Methods: Selected 811 inpatient cases with expensive costs whose total hospitalization expense of indi-vidual patients is over RMB100, 000 at a 3A hospital of Shanghai in 2014.Data is analyzed with SPSS 19.0.Results:Drugs and medical material costs are the highest in high hospitalization expenses.The influencing factors are including gender, medical payment, and classification of disea-ses, department, hospital days and curative effect.Conclusion: By lowering drugs and material costs, strengthen health education of focus groups, monitoring priority diseases and departments, reducing the average hospitalization days, improving medical standards, perfecting clinical pathways, exploring DRGs, the hospital can effectively control the high inpatients.
