股前外侧穿支皮瓣移植修复手部软组织缺损的临床研究进展

手部软组织缺损是创伤外科常见的病种之一,特别是合并血管、神经、肌腱、骨关节损伤时,处理难度较大.传统的修复方法多采用腹部及上肢皮瓣覆盖创面,但受区的感觉缺失和供区瘢痕暴露是此方法的一大弊端.随着显微外科技术的发展,股前外侧穿支皮瓣广泛应用于临床修复手部软组织缺损,并在临床实践中取得了满意的效果.了解股前外侧穿支皮瓣的解剖学特征以及修复手部软组织缺损时的适应证与优缺点,精准定位穿支位置对于提高手部软组织缺损的手术成功率至关重要.%Hand soft tissue defect is one of the most common diseases in traumatic surgery.It is especially difficult to handle when combined with vascular,nerve and tendon,bone and joint injuries.Traditional repair methods often use abdomi-nal and upper limb flaps to cover the wound,but the lack of sensation in the affected area and the scar exposure in the donor site are a major drawback of this method.With the development of microsurgical technique,anterolateral thigh perforator flap is widely used in repairing and reconstruction of soft tissue defect of hand,and satisfactory results have been achieved in clinical practice.Understanding the anatomical characteristics of anterolateral thigh perforator flap and the indications and advantages and disadvantages of repairing hand soft tissue defect,and accurate positioning of perforating branches are impor-tant for improving the success rate of hand soft tissue defects repair surgery.
