血清游离脂肪酸与非胰岛素依赖型糖尿病代谢异常关系探讨

Objective To investigate the relationship between serum free fatty acid(FFA)and non-insulin-dependent diabetes mellitus(NIDDM) metabolic disorder.Methods A total of 625 patients with NIDDM and 283 healthy subjects were enrolled,and serum FFA,total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),apolipoprotein(apo) A1,apo B100,lipoprotein [Lp(a)],homocysteine(Hcy),glucose(Glu),glycated hemoglobin A1c (HbA1c),uric acid(UA),creatinine(Cr),urea,cystatin C(Cys C)and alanine aminotransferase(ALT) were determined. NIDDM patients were classified into new-diagnosed NIDDM group(47 cases),stable group(545 cases) and unstable group(33 cases).Results There was statistical significance for these parameters between newly-diagnosed NIDDM and healthy control groups(P<0.05). There was statistical significance for FFA,Lp (a),apo A1,apo B100,LDL-C,HDL-C,Glu,ALT,HbA1c and UA between unstable and healthy control groups(P<0.01). There was statistical significance for HDL-C,apo A1,LDL-C,apo B100,FFA,Lp(a), Glu,ALT,HbA1c and UA between stable and healthy control groups(P<0.05),and there was no statistical significance for other parameters compared with healthy control group(P>0.05).Conclusions The levels of FFA and blood lipid in NIDDM patients have a correlation with metabolic disorder.%目的 探讨血清游离脂肪酸(FFA)与非胰岛素依赖型糖尿病(NIDDM)代谢的关系.方法 检测625例NIDDM患者、283名健康体检者(正常对照组)血脂{游离脂肪酸(FFA)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A1(apo A1)、载脂蛋白B100(apo B100)、脂蛋白(a)[Lp(a)]}、同型半胱氨酸(Hcy)、血糖(Glu)、糖化血红蛋白(HbA1c)、尿酸(UA)、肌酐(Cr)、尿素、胱抑素C(Cys C)及丙氨酸氨基转移酶(ALT)水平.将NIDDM患者按病情分为初发病组(47例)、病情稳定组(545例)及病情不稳定组(33例),对各组检测结果进行比较.结果 初发病组各项指标检测结果与正常对照组比较,差异均有统计学意义(P<0.05);病情不稳定组FFA、Lp(a)、apo A1、apo B100、LDL-C、HDL-C、Glu、ALT、HbA1c及UA与正常对照组比较,差异均有统计学意义(P<0.01),病情稳定组HDL-C、apo A1、LDL-C、apo B100、FFA、Lp(a)、Glu、ALT、HbA1c、UA与正常对照组比较,差异均有统计学意义(P<0.05),其他指标检测结果2个组间差异无统计学意义(P>0.05).结论 NIDDM患者FFA及血脂水平与其代谢异常有一定的关系.
