37例CD45阳性的多发性骨髓瘤浆细胞免疫学表型特征

摘要

目的 通过分析CD45阳性(CD45+)多发性骨髓瘤(MM)浆细胞的免疫表型特征及其与CD45阴性(CD45-)MM浆细胞免疫表型的区别,探讨CD45表达与MM临床特征的相关性.方法 回顾性分析192例初诊MM患者浆细胞的免疫表型,收集2个组的相关临床信息及实验室数据进行比对研究.结果 根据浆细胞CD45的表达情况将192例MM患者分为CD45+MM组(37例,浆细胞的CD45平均表达荧光强度>102)和CD45-MM组(155例).CD45+MM组浆细胞CD138、CD38、CD19、CD56、CD20、CD71、CD117、CD33的阳性率分别为83.8%、97.3%、32.4%、64.9%、21.6%、51.4%、35.1%、24.3%;胞浆κ轻链和λ轻链的阳性率均为45.9%,有8.2%不表达轻链.2个组浆细胞CD19、CD71和CD138的阳性率差异有统计学意义(P<0.05);其他抗原的阳性率及胞浆轻链的表达差异均无统计学意义(P>0.05).结论 CD45+MM患者浆细胞CD19和CD71阳性率明显升高,而CD138阳性率明显降低,推测其分化阶段可能处于浆细胞的偏早期.%Objective To investigate the plasmocyte immuno-phenotype characteristics of CD45-positive (CD45+) multiple myeloma(MM) and its difference with the plasmocyte immuno-phenotype characteristics of CD45-negative(CD45-)MM,and to analyze the correlation of CD45 expression with MM clinical characteristic. Methods The data of 192 MM patients were reviewed for immuno-phenotype analysis.Results According to CD45 expression,192 MM patients were classified into CD45+ MM(CD45 average fluorescence intensity on plasma cells >102) group(37 cases) and CD45-MM group(155 cases). In CD45+ MM group,the positive rates of CD138,CD38,CD19,CD56,CD20,CD71,CD117 and CD33 were 83.8%,97.3%,32.4%,64.9%, 21.6%,51.4%,35.1% and 24.3%. The positive rates ofκ andλ light chains were 45.9%. There was 8.2% with no light chain expression. The positive rates of CD19,CD71 and CD138 had statistical significance between the 2 groups (P<0.05). There was no statistical significance between the 2 groups for the positive rates and light chain expression of CD38,CD56,CD20,CD117 and CD33(P>0.05).Conclusions For CD45+ MM patients,their CD19 and CD71 positive rates increase,and CD138 positive rate decreases. CD45+ MM cells may be in the early differentiation stage of plasma cells.
