后路颈椎管单开门减压内固定治疗无骨折脱位型颈髓损伤

摘要

Objective To explore the mechanism of cervical spinal cord injury without fracture and dislocation and surgical outcome of posterior single-door decompression and internal fixation.Method The diagnosis and surgical treatment of 23 patients with cervical spinal cord injury without fracture and dislocation were analyzed retrospectively. The clinical manifestation and the change of Joa score of cervical spinal cord function were observed before and after operation.ResultThis group of 23 cases was followed up for 3 months to 2 years (18 months on average). The preoperative Joa score was 1~12 points, with an average of 7.2 points;the 12-week postoperative score was 3~16 points, with an average of 11.6 points. statistically significant differences were observed preoperatively and postoperatively in clinical manifestation and the change of Joa score of spinal cord function. limb muscle strength and motor function in 23 cases of patients after operation improved significantly than that before operation. ConclusionThe recovery of spinal cord function can be obtained in the early surgical treatment for the cervical spinal cord injury without fracture and dislocation.%目的：探讨无骨折脱位型颈髓损伤的受伤机制以及后路单开门减压内固定手术治疗效果。方法回顾性分析23例无骨折脱位型颈髓损伤的患者的诊断和手术治疗，观察手术治疗前后的临床表现和脊髓功能Joa评分的变化。结果本组23例术后随访3个月到2年，平均18个月，术前Joa评分为1～12分，平均分为7.2；术后12周评分为3～16分，平均为11.6分，术前术后评分二者有统计学显著性差异，术后23例患者四肢肌力及运动功能均较术前明显改善。结论对于无骨折脱位型颈髓损伤早期手术治疗能获得较为满意的脊髓功能恢复。
