3D打印模型在先天性高肩胛症儿童治疗中的应用

         

摘要

目的 观察3D打印双侧肩胛骨模型在先天性高肩胛症儿童治疗中的应用.方法 从2011年4月至2016年2月我科治疗先天性高肩胛症儿童10例,男3例,女7例;左侧4例,右侧6例.按照Cavendish分级,Ⅱ级2例,Ⅲ级6例,Ⅳ级2例.8例儿童在术前行双侧肩胛骨3D打印模型,2例儿童行双侧肩胛骨并锁骨、颈椎、胸廓全打印.所有儿童均行Woodward手术治疗.结果 10例患者中6例术前患侧肩胛骨形态学有明显改变,4例大体形态正常.4例有肩椎骨.术后随访6个月至5年10个月.术后按照Cavendish分级,Ⅰ级6例,Ⅱ级4例.6例随访时再次进行3D打印模型,并与术前对比.肩胛骨的高度和宽度均增加,大体形态没有明显变化.结论 3D打印肩胛骨模型可以直观地显示患侧肩胛骨与正常侧的差别,有助于术前的手术设计.3D立体打印肩胛骨模型较单纯肩胛骨模型能在空间上直观地显示肩胛骨与周围骨骼的关系,对术前的手术设计更为有利.%Objective To observe the application of 3D printing bilateral scapula model in the treatment of Sprengel deformity in children.Methods From April 2011 to February 2016,there were 10 cases of Sprengel deformity including 3 boys and 7 girls.The left side was affected in 4 cases,right side was affected in 6 cases.According to Cavendish classification,2 cases were grade Ⅱ,4 cases were grade Ⅲ,2 cases were grade Ⅳ.8 children were underwent bilateral scapula 3D printing model.2 children underwent bilateral scapula and clavicle,cervical vertebra,thoracic full print.All children underwent Woodward surgery.Results Preoperative ipsilateral scapula morphology changed obviously in 6 cases.The shapes of affected scapula were regular with the contralateral sides in 4 cases.4 cases had omovertebral bones.All cases were followed from 6 months to 5 years 10 months.According to Cavendish classification,6 cases were grade I,4 cases were grade Ⅱ postoperatively.6 cases were performed 3D printing model again.The height and width of the scapula increased,but there was no significant change in the gross morphology.Conclusion 3D model can display the differences about the ipsilateral scapula and the normal side directly.It is helpful in the preoperative design.The 3D printing scapula model can display the relationship between the affected scapula and the surrounding bones directly in space,which is more helpful in the preoperative design.
