目的 评价新型体位辅助用具对早产儿体位支持的有效性及早期神经行为发育的影响.方法 将71例胎龄<34周早产儿随机分为对照组34例与干预组39例,对照组应用常规体位护理方法,干预组应用自制新型体位辅助用具实行体位干预直至出院.于矫正胎龄34周、36周进行效果评价.结果 干预组生理性屈曲时间显著长于对照组,大腿僵硬外展、手臂外展时间显著短于对照组,突然滚动发生次数显著少于对照组,干预组矫正胎龄36周时出现睡眠觉醒周期次数显著高于对照组,头颅B超颅内出血发生率显著低于对照组(P<0.05,P<0.01).结论 新型体位辅助用具有利于早产儿维持生理性屈曲,减少不良姿势和四肢突然外展(惊吓)等不良运动的发生,有利于早期神经行为发育.%Objective To investigate the effects of a new positional support on postural stability and early neurobehavioral development in preterm infants.Methods Seventy-one preterm infants born at less than 34 weeks of gestation were randomized into a control group of 34 and an observation group of 39,receiving either routine postural care or a new positional support to maintain body posture.Effects were measured at the corrected age of 34 and 36 weeks.Results The observation group had a longer time of maintai ning physiological flexion,but a shorter time of thigh and arm extension,as well as less counts of sudden rolling than the control group (P<0.05,P<0.01).The observation group had more counts of sleep-wake cycle and lower incidence of cerebral hemorrhage at the corrected age of 36 weeks than the control group (P<0.05,P<0.01).Conclusion The new positional support could help preterm infants maintain physiological flexion,reduce poor posture and movements such as sudden extension induced by scare,thus to promote their early neurobehavioral development.
