MRI最小ADC值取代平均ADC值在宫颈癌诊断及分期中的应用研究

         

摘要

目的 评价MRI最小ADC值取代平均ADC值检查在宫颈癌诊断及分期的应用价值.方法 选取68例宫颈癌为研究组,20例正常者为对照,均行盆腔MRI检查,观察其平均表观扩散系数值(ADC)及最小ADC值,同时比较宫颈癌患者的FIGO分期与MRI分期的相关性.结果 宫颈癌病例组平均ADC值(1.0361±0.0194)×10-3 mm2/s,最小ADC值(0.910±0.0183)×10-3 mm2/s;对照组平均ADC值(1.873±0.0274)×10-3 mm2/s,对照组最小ADC值(1.760±0.0243)×10-3 mm2/s,两组平均ADC及最小ADC差异有显著统计学意义(P<0.05).患者MRI分期在显示宫旁、 盆壁受侵及远处转移方面均优于FIGO分期,MRI对二者区分的准确度为94%,对有无宫旁侵犯判断的准确度、特异度分别为92%、94%.结论 MRI最小ADC值能清晰显示宫颈癌肿瘤大小及侵犯范围,对宫颈癌的准确分期及治疗方案的制定提供依据,MRI可作为宫颈癌首选的影像学检查方法.%Objective To evaluate the of the minimum ADC value of MRI replaces the average ADC value in diagnosis and staging of cervical cancer. Methods 68 cases of cervical cancer were selected as the observation group, 20 cases were normal group, they all underwent pelvic MRI examination. We observed the mean ADC value and the minimum ADC value, and com-pared the FIGO staging and MRI staging of cervical cancer patients. Results The mean ADC value was (1. 0361 ± 0. 0194) × 10-3 mm2/s, the minimum ADC value was (0. 910 ± 0. 0183) × 10-3 mm2/s. The mean ADC value was (1. 873 ± 0. 0274) × 10-3 mm2/s in the normal group (1. 760 ± 0. 0243) × 10-3 mm2/s. There was significant difference between the two groups ( P <0. 05). Patients with MRI staging in the showed next to the uterine wall invasion and distant metastasis were better than FIGO staging. MRI for the distinction between the accuracy wsa 94%. The accuracy and specificity were 92% and 94% respectively. Conclusion MRI can clearly show the size of cervical cancer and the scope of violations, which provides a strong basis for the accurate staging of cervical cancer and the development of treatment programs. MRI can be used as the preferred imaging method for cervical cancer.
