二维联合三维超声在产前早孕晚期胎儿心脏畸形诊断中的应用研究

         

摘要

目的 探讨二维联合三维超声在产前早孕晚期胎儿心脏畸形诊断中的应用效果.方法 选取我院检查就诊的早孕晚期孕产妇200例,对孕产妇通过二维联合三维超声对孕产妇进行超声心动图检查,比较分析产前超声诊断结果和产后结果.结果 本次纳入研究的212例胎儿中,生产前通过二维联合三维超声检查患有先天性心脏畸形的胎儿有70例,经产后回访结果为78例,诊断符合率为89.74%.78例胎儿中严重畸形的胎儿有58例,占胎儿心脏畸形的74.36%,非严重畸形的胎儿(室间隔缺损)20例,占胎儿心脏畸形的25.64%.单纯4CV切面诊断胎儿先天性心脏畸形152例,占胎儿总数的71.69%.结论 在产前早孕晚期心脏畸形胎儿筛查中应用二维联合三维超声检查具有较高的诊断准确率,且具有应用方便、操作简单、无创伤.%Objective To study the efficiency of two-dimensional combined with three-dimensional ultrasound in the diagnosis of fetal cardiac malformation in late pregnancy prenatally. Methods 200 pregnant and parturient women, who visited our hospital for prenatal inspection from May 2014 to January 2017, were selected in this work. Through two-dimensional combined with three-dimensional ultrasound, the echocardiography was conducted and the results of the prenatal ultrasound and postpartum diagnosis were compared and analyzed. Results Of those 212 fetuses, 70 were detected with congenital heart malformation via two-dimen-sional joint three-dimensional ultrasound before the birth, while, the postpartum visit results were 78 cases. Thus, the diagnostic coincidence rate was 89. 74%. Among those 78 fetuses, 58 presented with severe malformation, accounting for 74. 36% of fetal cardiac malformations, 20 were non-serious malformation fetus (VSD), accounting for 25. 64% of the fetal cardiac malforma-tion. Only 4 CV section diagnosed 152 cases with the fetal congenital heart malformation, accounting for 71. 69% of the fetuses. Conclusion The two-dimensional combined with three-dimensional ultrasound has high diagnostic accuracy in prenatal screening for cardiac malformation fetuses at late pregnancy. This strategy has the traits in terms of the convenient application, simple opera-tion, no trauma, and easy acceptation by the pregnant women. At the same time, to provide diagnostic accuracy, it should strengthen all aspects of joint detection, in order to better improve the detection rate of fetus congenital heart diseases.
