肝癌组织中HIF-1α、VEGF表达水平及临床意义

摘要

目的 分析原发性肝癌(PHC)组织中缺氧诱导因子-1α(HIF-1α)和血管内皮生长因子(VEGF)的表达情况及临床意义.方法 收集我院2011年9月至2016年12月期间行手术切除治疗并经病理证实的60例PHC患者的蜡块组织标本,其中有29例患者包含有癌旁组织,作为PHC组织的对照.所有组织标本均进行免疫组化染色,根据阳性细胞的染色强度和阳性细胞所占百分比的乘积判定HIF-1a、VEGF表达情况,并分析其与病理特征及预后的关系.结果 PHC组织HIF-1a、VEGF阳性表达率依次为75.00％、70.00％,显著高于癌旁组织阳性表达率的0.00％、3.33％ (P ＜0.05).在PHC组织中,HIF-1a、VEGF的阳性表达呈正相关(r=0.61,P＜0.05).PHC患者性别、年龄、肝功能分级、肿瘤大小、肿瘤分化程度与HIF-la、VEGF表达无明显相关性(P＞0.05),但PHC分期、转移情况与HIF-1a、VEGF表达具有显著相关性(P＜0.05).HIF-1a阳性、阴性表达者中位无病生存期分别为28.1、41.5个月,差异有统计学意义(P＜0.05);VEGF阳性、阴性表达者的中位无病生存期分别为16.4、34.8个月,差异有统计学意义(P＜0.05).结论 HIF-1α、VEGF在PHC组织中的表达与肿瘤分期、转移情况密切相关,并可作为预测患者术后无瘤生存期的重要指标.%Objective To analyze the expression of hypoxia inducible factor-1α (HIF-1α) and vascular endothelial growth factor (VEGF) and theirs chnical significance in primary hepatic carcinoma (PHC) tissues.Methods The wax block tissue specimens of 60 cases of PHC patients treated with surgical resection and pathologically confirmed in our hospital from September 2011 to December 2016 were collected for the study,including 29 patients with adjacent tissues taken as the contrast of PHC tissues.All tissues were immunohistochemically stained.And the expression of HIF-1 α and VEGF was determined according to the product of positive cells staining intensity and the percentage of positive cells,and theirs relationship with pathological features and prognosis was analyzed.Results The positive expression rates of HIF-1α and VEGF in the PHC tissues were 75.00％ and 70.00％ respectively,which were significantly higher than those in the adjacent tissues with0.00％ and 3.33％ (P ＜ 0.05).There was a positive correlation between the positive expression of HIF-1 α and VEGF in the PHC tissues (r =0.61,P ＜ 0.05).There was no significant correlation between the gender,age,liver function grading,tumor size,tumor differentiation and the expression of HIF-1 α and VEGF (P ＞ 0.05),but there was a significant correlation between PHC staging and metastasis and the expression of HIF-1 α and VEGF (P ＜ 0.05).The median disease -free survival times of the patients with HIF-1α positive and negative expression were 28.1 and 41.5 months respectively,(P ＜0.05).The median disease-free survival times of the patients with VEGF positive and negative expression were 16.4 and 34.8 months respectively (P ＜ 0.05).Conclusions The expression of HIF-1 α and VEGF in PHC tissues is closely related to tumor staging and metastasis,and can be used as an important index to predict postoperative disease-free survival.
