血清中炎性细胞因子在类风湿关节炎患者中的检测及临床意义

[ABSTRACT]Objective:To investigate the relationship between inflammatory cytokines and rheumatoid arthritis (RA), and to explore the clinical application value of cytokines in diagnosis,treatment and prognosis of RA.Methods:A total of 136 patients with RA served as RA group,including 62 cases in remission and 74 cases in active period;53 healthy volunteers served as control group.The changes of interleukin (IL)-1β,IL-6,IL-8,IL-10 and interferon (IFN)-r in each group were compared.Also,the correlation between inflammatory cytokines and DSA28 score as well as C-reactive protein in patients was studied.Results:The comparison of IL-1βand IFN-r between patients in remission in RA group and in control group showed no significant difference (P >0.05),while statistically significant differences of IL-1βand IFN-r presented in patients in active period in RA group,compared with patients in remission in RA group and control group (P <0.05).Comparison of IL-6,IL-8,IL-10 between any two groups showed statistical differences (P <0.05);C-reactive protein,DSA28 score and IL-6,IL-8, IL-10 were positively correlated,while IL-1β,IFN-r and CRP,DSA28score showed no correlation.Conclusions:The detection of inflammatory cytokines carries important clinical significance in the early diagnosis,evaluation and prognosis of RA.%目的：研究炎性细胞因子与类风湿性关节炎关系,探讨细胞因子作为类风湿性关节炎诊断、治疗及预后的临床应用价值。方法：136例类风湿性关节炎患者设为类风湿性关节炎组,其中62例处于缓解期,74例为活动期,53例健康志愿者为对照组,比较各组中白介素-1β(IL-1β)、白介素-6(IL-6)、白介素-8(IL-8)、白介素-10(IL-10)、干扰素-r(IFN-r)的变化情况,并研究类风湿性关节炎患者炎性细胞因子与 DSA28评分及 C 反应蛋白相关性。结果：类风湿性关节炎组缓解期 IL-1β、IFN-r 与对照组比较,差异无统计学意义(P >0.05),类风湿性关节炎组活动期 IL-1β、IFN-r 与对照组、类风湿性关节炎组缓解期均有统计学差异(P <0.05),IL-6、IL-8、IL-10各组两两比较均存在统计差异(P <0.05)；CRP、DSA 评分与 IL-6、IL-8及 IL-10均呈正相关,6IL-1β、IFN-r 与 CRP、DSA 评分无相关性。结论：炎性细胞因子的检测于类风湿性关节炎早期诊断、病情评估及预后预测均有重要的临床意义。
