胸腔镜下同期双侧与序贯双侧肺减容术治疗肺气肿比较

摘要

Objective: To compare the effect of the same term and sequential lung volume reduction by video assisted thoracoscopic lung volume reduction surgery (LVRS) in chronic obstructive pulmonary disease(COPD) of heterogeneity.Methods: The clinical data of LVRS cases with severe COPD performed by video assisted thoracoscopy from January 2010 to December 2013 was retrospectively analyzed. Lung function、blood gas analysis、cardiac function、time of drainage tube were included. Results: The improvement of FEV1 、PaO2、,PaCO2、CO are obviously in patients after sequential lung volume reduction surgery. The same term lung volume reduction is better than sequential one.Conclusion: Video assisted thoracoscopic LVRS brings improvement of lung function and cardiac function of patients with COPD. The same term lung volume reduction is better than sequential one.%　　目的对比胸腔镜下同期双侧肺减容术与序贯双侧肺减容术对肺气肿患者术后肺功能、心功能改善。方法对我院2010.1-2013.12胸腔镜手术治疗肺气肿患者进行回顾性研究，对患者术前、术后肺功能指标、血气分析指标、术后心功能、带管时间行回顾研究。结果25例序贯双侧肺减容患者术后FEV1明显改善，PaO2明显升高（P ﹤0.05）,PaCO2明显降低（P ﹤0.05）,CO升高（P >0.05）,同期双侧肺减容患者要优于序贯肺减容患者。结论胸腔镜下肺减容手术对患者肺功能、心功能有改善，同期双侧肺减容优于双侧序贯肺减容。
