分期针刺配合推拿对脑卒中患者认知功能及血清NSE、S1OOB蛋白的影响

摘要

目的 观察分期针刺配合推拿对脑卒中患者认知功能的影响及血清神经元特异性烯醇化酶(NSE)、S100B蛋白的影响.方法 将116例脑卒中患者按照随机数字表法分为研究组和对照组各58例,两组均给予控制血压、调节血糖等对症治疗,在此基础上对照组给予推拿治疗,研究组在对照组基础上配合分期针刺治疗.观察两组中医症状积分、认知功能、运动功能、日常生活能力、神经功能缺损程度、NSE和S100B蛋白水平.结果 治疗后两组中医症状积分低于治疗前(P＜0.05),且研究组低于对照组(P＜0.05);治疗后两组思维运作、定向、视运动组织、知觉评分高于治疗前(P＜0.05),且研究组高于对照组(P＜0.05);治疗后两组Fugl-Meyer运动量表(FMA)评分、日常生活能力量表(ADL)评分、美国国立研究院卒中量表(NIHSS)评分优于治疗前(P＜0.05),且研究组优于对照组(P＜ 0.05);治疗后两组NSE、S100B蛋白水平低于治疗前(P＜0.05),且研究组低于对照组(P＜0.05).结论 分期针刺配合推拿治疗能够有效降低脑卒中患者血清NSE和S100B蛋白水平,改善神经功能和运动功能,提高认知功能.%Objective:To analyze the influence of staging acupuncture combined with massage therapy on cognitive function and changes of serum NSE and S100B of stroke patients.Methods:116 cases of stroke patients were selected as the study objects,and were divided into the study group and the control group according to random number table method,58 cases in each group.Both groups were given blood pressure control,blood sugar regulation and other symptomatic treatment,and the control group were given massage therapy on the basis of that,and the study group were combined with staging acupuncture on the basis of the control group.TCM symptom scores,cognitive function,motor function,daily living ability,neurological deficit,NSE and S100B levels were observed in the two groups.Results:After treatment,TCM symptom scores were significantly lower than those before treatment in the two groups (P＜0.05),and those of the study group were significantly lower than those of the control group (P＜ 0.05).Thinking operation,orientation,visual organization and perceptual scores were significantly higher than those before treatment in the two groups (P ＜ 0.05),and those of the study group were significantly lower than those of the control group(P＜ 0.05).After treatment,FMA,ADL and NIHSS scores were significantly higher than those before treatment in the two groups (P＜ 0.05),and those in the study group were significantly higher than those in the control group(P＜0.05).NSE and S100B levels were significantly lower than those before treatment in the two groups (P＜ 0.05),and those in the study group were significantly lower than those in the control group(P＜0.05).Conclusion:Staging acupuncture combined with massage therapy can effectively reduce serum NSE and S100B levels of stroke patients,improve neurological function and motor function,and improve cognitive function.
