自我管理理念在溃疡性结肠炎护理中的应用分析

摘要

Objective To Investigate the effect of health education based on self-management concept on intestinal symptoms in patients with ulcerative colitis.Methods 90 patients with ulcerative colitis treated in our hospital from March 2014 to May 2016 were recruited and assigned to treatment group and control group using random number table,with 45 patients in each group.The treatment group received health education program based on a self-management based on the concept,and the control group received routine health education.The total score of various intestinal symptoms,psychological harmony state total score,quality of life total score before and 2 months after nursing,and the recurrence rate at 1-year follow-up were compared between two groups.Results The scores of abdominal pain,diarrhea,blood in the stool and mucus stool 2 months after nursing in treatment group were lower than before nursing,and lower than in the control group (P ＜ 0.05).The total score of psychological harmony and IB-DQ score in treatment group were higher than before nursing,and higher than in the control group (P ＜ 0.05).The recurrence rate was 6.67％ in treatment group during the follow-up period,which was lower than that in the control group (22.22％,P ＜ 0.05).Conclusion The application of self-management in the nursing of patients with ulcerative colitis is helpful to alleviate intestinal symptoms,improve the mental state,improve patients' quality of life and reduce recurrence rate.%目的 观察自我管理理念在溃疡性结肠炎患者护理中的应用价值.方法 选取本院2014年3月至2016年5月90例溃疡性结肠炎患者作为研究对象,随机分为观察组与对照组,每组各45例.观察组采取基于自我管理理念的健康教育,对照组采取常规健康教育.比较两组护理前、护理2个月后的各肠道症状总评分、心理和谐状态总评分、生活质量总评分及随访1年的复发率.结果 观察组护理2个月后的腹痛、腹泻、便血、黏液便评分均低于本组护理前及对照组(均P＜ 0.05),心理和谐状态总评分与IB-DQ总评分均高于本组护理前及对照组(均P＜0.05).观察组随访期间复发率6.67％,低于对照组的22.22％(P＜ 0.05).结论 自我管理理念在溃疡性结肠炎患者护理中的应用有利于缓解患者肠道症状及改善心理状态,促进患者生活质量的提高,降低复发率.
