虚挂线法治疗高位肛瘘的临床效果观察

摘要

目的 探讨虚挂线法治疗高位肛瘘的临床疗效.方法 将2015年4月至2016年6月四川省绵阳市肛肠病医院肛肠科收治的120例高位肛瘘患者随机分成观察组与对照组,每组各60例.观察组采用虚挂线法治疗,对照组采用传统挂线法治疗.比较两组术后创面愈合时间、瘢痕组织面积、肛门功能、疼痛评分、总有效率及1年内复发率.结果 观察组患者创面愈合时间平均为(26.81±2.43)d,对照组为(27.64±2.26)d,两组差异无统计学意义(P＞0.05);两组总有效率差异无统计学意义,术后1年内复发率均为16.7％,差异无统计学意义(均P＞0.05).观察组瘢痕组织面积小于对照组、疼痛评分及肛门功能等方面优于对照组,差异均有统计学意义(均P＜ 0.05).结论 虚挂线法治疗高位肛瘘与传统挂线疗法相比具有瘢痕小、疼痛轻、肛门功能保护较好等优点,有一定的临床推广价值.%Objective To investigate the clinical effectiveness of minimally invasive loose seton in the treatment of high anal fistula.Methods 120 patients with high anal fistula were randomly assigned into two groups,with 60 patients in each group.The treatment group was treated with minimally invasive loose seton and the control group with the traditional seton.Postoperative time to wound healing,total area of scar tissue,anal function,pain score,total efficiency of treatmrnt and 1-year recurrent rate were compared between the two groups.Results Average time to wound healing was (26.81±2.43)days in the treatment group and(27.64±2.26)days in the control group,with no significant difference between the two groups (P ＞ 0.05).There was no difference between the two groups in total efficiency of treatment (P ＞ 0.05).Overall 1-year recurrent rate was 16.7％ in both groups,with no significant difference between the two group (P ＞ 0.05).The total area of scar tissue,pain score,and anal function were significantly better in the treatment group than that in the control group (all P ＜ 0.05).Conclusion The minimally invasive loose seton is associated with smaller scar,less pain and better anal function than traditional seton.It is a preferred method for the treatment of high anal fistula.
