【目的】了解社区中老年人高血压患病率及药物使用情况，为制定社区高血压防治措施提供参考依据。【方法】采取整群抽样的方法于2014年1～6月对上海市杨浦区大桥街道45～85岁常住居民进行问卷调查和血压测量，并对高血压患者用药情况和用药依从性进行调查分析。【结果】该社区高血压患病率为34．77％（1467／4219），76～85岁年龄段患病率显著高于其他年龄段，差异有统计学意义（ P ＜0．05）；高血压患者使用较多的药物为钙通道阻滞剂（CCB）类药物（39．60％）、血管紧张素转换酶抑制剂（ACEI）类药物（25．49％）和复方降压剂（22．97％）；高血压患者单一用药比例最高，占56．51％；服药依从性良好患者803例，占54．74％；学历水平、健康教育经历、病程、每天用药次数对高血压患者依从性有显著影响（ P ＜0．05）。【结论】76～85岁年龄段高血压患病率处于较高水平，整体用药较单一，服药依从性处于一般水平，以CCB降压药物为主，利尿剂类应用较少，针对以上现状很有必要开展综合健康教育，以提高社区中老年高血压患者血压控制率。%[Objective] To explore the incidence and medications in middle age and elderly patients with hypertension in communities to provide rationales for community hypertension controls .[Methods]From Jan‐uary to June 2014 ,the Bridge Street residents of Yangpu District aged 45~85 were recruited to measure blood pressures and responded to a survey of hypertension medication and medication compliance .[Results] The in‐cidence of hypertension was 34 .77% (1467/4219) .For 76~85 year group ,the incidence rate of 49 .65% was significantly higher than those of other age groups .And the differences were statistically significant ( P <0 .05) .And the categories were calcium channel blockers (39 .60% ) ,angiotensin‐converting enzyme inhibitors (25 .49% ) and compound antihypertensive agents (22 .97% ) .Monodrug regimens accounted for 56 .51% . The medication compliance was good in 803 cases (54 .74% ) .And the influencing factors of hypertension med‐ication included disease course ,health education ,education level and daily dosing frequency ( P < 0 .05) .[Conclusion] The 66~75 year group has a high incidence of hypertension .The number of drug is mostly sin‐gle and there is a fair overall compliance .Calcium antagonists are preferred over diuretics .Therefore it is nec‐essary to provide comprehensive health education for improving blood pressure control in elderly hypertensive patients in communities .
