冠状切口双额开颅去骨瓣减压术治疗双额叶重型脑挫裂伤21例

摘要

目的：：探讨双额开颅去骨瓣减压术治疗双额叶重型脑挫裂伤疗效。方法：选择双额叶重型颅脑损伤患者21例，均行冠状切口双额开颅去骨瓣减压术，不保留额骨骨桥。结果：术后随访1年，存活19例，死亡2例；根据格拉斯哥预后评分评价疗效：良好11例(52.3%)，轻残5例(23.8%)，重残3例(14.2%)，死亡2例(9.5%)。结论：双额开颅去骨瓣减压术对双侧额叶重度脑挫伤患者手术效果确切，可有效改善患者预后，降低致残率和死亡率。%Objective:To explore the curative effects of double frontal decompressive craniectomy through coronal incision in the treatment of severe bifrontal contusion. Methods:The data of 21 patients with severe bifrontal contusion treated with double frontal decompressive craniectomy through coronal incision and without retaining frontal bone bridge were retrospectively analyzed. Results:All patients were followed up for 1 year. Nineteen cases survived,2 cases died. Excellent in 11 cases(52. 3% ),light disability in 5 cases (23. 8% ),heavy disability in 3 cases(14. 2% ) and death in 2 cases(9. 5% ) were found according to the Clasgow outcome scale. Conclusions:The curative effects of double frontal decompressive craniectomy through coronal incision in treating severe bifrontal contusion are good,which can effectively improve the prognosis,and reduce disability and mortality of patients.
