泻阴补阳针刺法联合化痰通络解痉汤治疗脑卒中痉挛性瘫痪

         

摘要

目的 探讨泻阴补阳针刺法联合化痰通络解痉汤内服治疗脑卒中痉挛性瘫痪的临床疗效.方法 选取我院针灸科收治的脑卒中后痉挛性瘫痪患者186例,按随机数字表法分组,对照组93例予以泻阴补阳针刺法治疗,研究组93例在对照组基础上予以化痰通络解痉汤治疗.结果 对照组治疗有效率(76.34%)低于研究组治疗有效率(88.17%),具有统计学意义(P<0.05);与对照组比较,研究组治疗后AShworth量表评分、CSI评分较低,Fugl-Meyer评分、Barthel指数较高,治疗后血清Glu、Glu/GABA比值水平较低,血清GABA水平较高,治疗后全血比黏度、血浆比黏度、还原比黏度及红细胞聚集指数较低,治疗后PT、APTT、TT水平较高,FIB水平较低,具有统计学意义(P<0.05).结论 泻阴补阳针刺法联合化痰通络解痉汤内服治疗脑卒中痉挛性瘫痪的临床疗效确切,能有效减轻痉挛,提高患者运动功能、日常生活能力,可能与其调节血清Glu、GABA含量,改善血黏度及凝血功能的作用有关.%Objective To investigate the clinical effect of acupuncture method of draining yin and nourishing yang combined with Huatan Tongluo decoction in the treatment of spastic paralysis after stroke. Methods 186 patients with spastic paralysis after stroke were selected in the acupuncture department of our hospital, and they were divided into groups by random number table method.93 cases in the control group were treated with the method of draining yin and nourishing yang , while 93 cases in the study group were treated with Huatan Tongluo decoction on the basis of the control group. Results The effective rate in the control group (76.34%)was lower than of the experiment group(88.17%) with statistical significance (P <0.05); compared with the control group, AShworth scale score and CSI score were lower, Fugl-Meyer score and Barthel index were higher after treatment in the study group, serum levels of Glu、Glu/GABA ratio were lower, serum level of GABA was higher after treatment, whole blood specific viscosity,plasma specific viscosity, reduction ratio viscosity and erythrocyte aggregation index were lower after treatment,levels of PT、APTT、TT were higher, level of FIB was lower after treatment,with statistical significance (P <0.05).Conclusion The clinical efficacy of acupuncture method of draining yin and nourishing yang combined with Huatan Tongluo decoction on spastic paralysis after stroke is effective,which can effectively relieve spasm, improve patients'motor function and daily life ability may be related to the regulation of serum Glu, GABA content, related to improve the function of blood viscosity and coagulation function.
