人卵裂期胚胎玻璃化冷冻复苏后的体外培养时间对妊娠结局的影响

摘要

目的 分析人卵裂期胚胎玻璃化冷冻复苏后的体外培养时间对妊娠结局的影响.方法 对本中心2014年12月至2017年9月行FET(冻融胚胎移植)的患者984个周期进行回顾性分析,以移植时间为依据,分为两组,甲组解冻后次日移植(540个周期),乙组解冻当天移植(444个周期),以甲组胚胎生长情况为依据,将甲组分为两个亚组,甲1组所有胚胎均生长(342个周期),甲2组部分胚胎生长(198个周期).对各组胚胎着床率、临床妊娠率、胚胎复苏率、移植胚胎数、内膜厚度等进行对比分析.结果 甲、乙两组移植胚胎个数、内膜厚度、患者年龄比较,差异均无统计学意义(均P> 0.05);与乙组对比,甲组种植率、妊娠率有升高趋势,但差异均无统计学意义(均P> 0.05);甲1组胚胎着床率高于甲2组,差异有统计学意义(P<0.05).结论 人卵裂期胚胎玻璃化冷冻复苏后的适当延长体外培养时间观察其发育潜能不会对妊娠结局产生不利影响,且延长培养时间后继续生长的胚胎具有更高着床率.%Objective To analyze the effect of in vitro culture time on pregnancy outcomes after vitrification and resuscitation of human cleavage stage embryos.Methods The 984 cycles of the patients taking freeze-thawed embryo transfer at our center from December,2014 to September,2017 were retrospectively analyzed,and were divided into group A (540 cycles) and group B (444 cycles) according to the transfer time.Group A were transferred the day after vitrification and resuscitation,and group B on the day of vitrification and resuscitation.Based on the embryo growth of group,group A was divided into two subgroups.All the embryos in the first group (342 cases) grew,but just part of the embryos in the second group (198 cases) grew.The embryo implantation rate,clinical pregnancy rate,embryo resuscitation rate,number of transplanted embryos,and intima thickness were compared and analyzed.Results There were no statistical differences in the number of embryos,embryo resuscitation rate,intimal thickness,and age between group A and group B (all P > 0.05).The implantation rate and pregnancy rate were higher in group A than in group B,but with no statistical difference (both P > 0.05).The embryo implantation rate was higher in the first group than in the second group,with a statistical difference (P < 0.05).Conclusions Prolonged in vitro culture time after vitrification and resuscitation of human cleavage stage embryos will not affect the pregnancy outcomes,and the embryos that continue to grow after prolonged culture time have higher implantation rate.
