阻塞性睡眠呼吸暂停相关性高血压与心律失常的研究

摘要

Objective To explore the correlation between obstructive sleep apnea related hypertension (OSA-RH) and arrhythmia.Methods 120 patients with OSA-RH treated in our hospital from November 2015 to November 2017 were selected as the observation group,and 120 patients with primary hypertension treated in our hospital during the same time period were selected as the control group,and the clinical indexes and arrhythmia occurrence of the two groups were compared.Results There were no statistically significant differences in TC,TG,hs-CRP,RBC,and serum creatinine between the control group and the observation group (P＞0.05);BMI,AHI,MA/Cr,left atrial diameter,and left ventricular diameter in the observation group were significantly higher than those in the control group,with statistically significant differences (P＜0.05).The incidence of arrhythmia in the observation group was significantly higher than that in the control group (65.00％ vs.18.33％) (P＜0.05);the incidence of arrhythmia in the mild,moderate,and severe patients of the observation group were 33.33％,50％,and 84.38％ respectively,showing an upward trend.Conclusion Compared with simple hypertension,the incidence of arrhythmia in obstructive sleep apnea related hypertension is higher,and the incidence of arrhythmia increases gradually with disease aggravation,related to the changes of BMI,AHI,MA/Cr,left atrium diameter,and left ventricular inner diameter,so monitoring should be strengthened and intervention treatment should be carried out in time.%目的 探讨阻塞性睡眠呼吸暂停相关性高血压(OSA-RH)与心律失常的相关性.方法 将2015年11月至2017年11月期间本院收治的阻塞性睡眠呼吸暂停相关性高血压患者120例作为观察组研究资料,另选择同时间段本院收治的原发性高血压患者120例作为对照组研究资料,比较两组临床指标及心律失常发生情况.结果 对照组与观察组TC、TG、hs-CRP、RBC、血肌酐比较差异无统计学意义(P＞0.05);与对照组比较,观察组BMI、AHI、MA/Cr、左心房内径、左心室内径均偏高,差异有统计学意义(P＜0.05);观察组心律失常发生率为65.00％,显著高于对照组的18.33％,差异有统计学意义(P＜0.05);观察组轻度、中度和重度患者发生率分别为33.33％、50.00％、84.38％,呈上升趋势.结论 与单纯高血压相比,阻塞性睡眠呼吸暂停相关性高血压心律失常发生率偏高,且伴随病情加重发生率逐渐增高,且发生BMI、AHI、MA/Cr、左心房内径、左心室内径等变化有关,需加强监测,及时进行干预处理.
