社区-医院糖尿病教育模式效果观察

摘要

Objective To observe the effect of community-hospital education mode for diabetes patients.Methods Professional doctors and nurses had been sent to community,using general education in community and individual education in hospital,including basic knowledge,diet control,exercise therapy,self monitoring,drug knowledge.Observed the changes of blood glucose,HbAlc,and compliance.Result There were statistically significant differences in FBG,2hPBG,HbAlc,treatment compliance before and after education (P＜0.05).Conclusion Community-hospital education mode for diabetes patients has good effect.%目的 探讨社区-医院糖尿病教育模式如何影响患者治疗的效果.方法 由医院派遣专业的医护人员在社区内开展糖尿病普及教育,延伸到医院进行个体化教育,包括糖尿病基础知识、饮食控制、运动疗法、自我监测、药物知识等,比较患者在接受教育前后的血糖值、糖化血红蛋白比值和就诊依从性.结果 教育后空腹血糖、餐后2小时血糖、糖化血红蛋白比值及就诊依从性与教育前相比差异有统计学意义(P＜ 0.05).结论 社区-医院糖尿病教育模式可以更好地控制血糖,提高患者就诊依从性.
