胃镜下联合应用金属止血夹和尼龙绳套扎治疗消化性溃疡并出血的疗效和安全性分析

摘要

Objective To investigate the of efficacy and safety of endoscopic hemostasis metal clip and nylon rope ligation in the treatment of peptic ulcer bleeding.Methods 150 cases of peptic ulcer bleeding in our hospital were randomly divided into three groups,metal hemostatic clip group,nylon rope ligation group,and combination group,50 patients in each group.Compared hemostasis efficiency,immediate hemostasis rate,the rate of turning into surgery,rebleeding rate of three groups.Results Hemostasis efficiency,immediate hemostasis rate,the rate of turning into surgery,rebleeding rate 72 h later of combination group were significantly higher than those of metal hemostatic clip group and nylon rope ligation group,with statistically significant differences (P＜0.05).There were no statistically significant differences in hemostasis efficiency,immediate hemostasis rate,the rate of turning into surgery,rebleeding rate 72 h later between metal hemostatic clip group and nylon rope ligation group (P＞0.05).Conclusions Compared with single metal hemostatic clip or nylon rope iigation treatment,the combined treatment of metal hemostatic clip and nylon rope ligation in patients with peptic ulcer bleeding can improve patients' hemostasis efficiency,immediate hemostasis rate,and reduce the rate of turning into surgery,rebleeding rate,with important clinical value.%目的 探讨胃镜下联合应用金属止血夹和尼龙绳套扎治疗消化性溃疡并出血的疗效及安全性.方法 选取本院收治的150例消化性溃疡并出血患者,将其随机分为金属止血夹组、尼龙绳套扎止血组和联合组各50例,比较治疗后三组患者的止血有效率、即时止血率、转外科手术率、再出血率等.结果 联合组的止血有效率、即时止血率、外科手术发生率、72 h后再出血率与金属止血夹组和尼龙绳套扎止血组比较差异有统计学意义(P＜0.05);金属止血夹组和尼龙绳套扎止血组的止血有效率、即时止血率、外科手术发生率、72 h后再出血率比较差异无统计学意义(P＞0.05).结论 与单用金属止血夹、尼龙绳套扎治疗比较,在消化性溃疡并出血患者中联合应用金属止血夹和尼龙绳套扎治疗能有效提高病人的止血有效率、即时止血率,降低转外科手术率、再出血率,具有重要的临床应用价值.
