改进型负压封闭引流治疗老年难愈性创面的临床研究

摘要

目的 探讨改进型负压封闭引流(VSD)治疗老年难愈性创面的效果.方法 将78例老年难愈性创面患者按照数字表法随机分为两组,对照组39例给予常规VSD治疗,观察组39例采用改进型VSD治疗,比较两组疗效.结果 观察组的总有效率为94.87％,明显高于对照组的79.49％,差异有统计学意义(x2=4.1294,P＜0.05).观察组并发症发生率5.13％(2/39),而对照组为20.51％(8/39),两组比较差异有统计学意义(P＜ 0.05).观察组上皮化时间为(3.4±0.8)d,而对照组为(6.1±1.1)d;观察组治疗时间为(11.59±2.43)d,而对照组为(18.02±2.46)d;两组比较差异有统计学意义(P＜0.05).结论 改进型VSD有助于改善创面微循环,引流坏死液化的组织和渗出液,有效降低了各种有害代谢产物进入血循环,同时促进肉芽组织生长,促进创面愈合,缩短住院时间,提高治疗效果,是一种简单、经济、安全有效的理想方法,值得在临床上推广应用.%Objective To explore the effect of improved vacuum sealing drainage (VSD) in the treatment of elderly patients with refractory wounds.Methods 78 elderly patients with refractory wounds were randomly divided into two groups in accordance with digital table method,39 cases in control group were given conventional VSD treatment,39 cases in observation group were given improved VSD treatment,and the therapeutic effect was compared between two groups.Results The total efficiency was 94.87％ in observation group,obviously higher than 79.49％ in control group,with statistically significant difference (x2=4.1294,P＜0.05).The incidence of complications was 5.13％ (2/39) in observation group and 20.51％ (8/39) in control group,with statistically significant difference (P＜0.05).The epithelization time was (3.4±0.8)d in observation group and (6.1± 1.1)d in control group,the treatment time was (11.59±2.43)d in observation group and (18.02±2.46)d in control group,with statistically significant differences (P＜0.05).Conclusions Improved VSD is helpful to improve the microcirculation of the wound,drainage liquefied necrotic tissue and exudate,effectively reduces harmful metabolites into blood circulation,and promotes the growth of granulation tissue,wound healing,shorten the hospitalization time,improve the therapeutic effect,which is a simple,economical,safe,effective,and ideal method,worthy of clinical application.
