新型超高频彩超在眼球屈光系统疾病诊治中的应用与价值研究

摘要

Objective To investigate the value of the new ultra-high frequency color Doppler ultrasound (UHF color Doppler ultrasound) in the diagnosis and treatment of ocular refractive system diseases.Methods We randomly selected 200 ophthalmic patients from our hospital.The eyeballs and their appendages of each patient were examined by 4 frequently used ophthalmic imaging instruments-new UHF color Doppler ultrasound and pulse Doppler (14-20 MHz CDFI and PWFI),fundus color photography,optical coherence tomography (OCT),and slit lamp microscope.All diagnostic results and data were divided into 4 groups and a comparative analysis for refractive system diseases was performed.Results Compared with other commonly used inspection methods,the positive rate,the clinical coincidence rate,and the adverse reaction rate of the first group (the new UHF color Doppler ultrasound examination group) were the best.In the first group,the positive rate and clinical coincidence rate of cataracta,vitreous body disease,and keratonosus were 100.00% and the adverse reaction rate was 0.00%.Furthermore,Compared with the other 3 groups,new UHF color Doppler ultrasound had obvious advantages in some aspects and could detect ocular refractive diseases at early stage.Conclusions The main component of ocular refractive system is water,which provides a good physical background plate for ultrasound and gives a high contrast to various eyeball tissues on the ultrasonic screen.The higher the ultrasonic frequency,the better the contrast ratio and clarity.Therefore,in recent years,ultra-high frequency color Doppler ultrasound (14-20 MHz) is an important examination method for ophthalmic diseases,especially for ocular refractive system disease.%目的 探讨新型超高频彩超在眼球屈光系统疾病诊治中的价值.方法 在本院求诊的眼科患者中,随机抽取200例,每例患者都分别使用新型超高频彩色超声多普勒及脉冲多普勒检查(14MHz~20 MHz CDFI及PWFI)、眼底照相、光学相干成像(OCT)、裂隙灯显微镜4种常用的眼科影像学仪器对眼球及其附属器进行检查,把所得诊断结果和数据分成相应四组进行屈光系统疾病方面数据的综合对照对比分析.结果 对比结果显示,相对其他常用的检查手段,第一组(新型超高频彩超检查组)在眼球屈光系统疾病的阳性检出率、临床符合率、不良反应率方面表现最优异,其中白内障、玻璃体疾病、角膜病的阳性检出率、临床符合率为100.00%,不良反应率为0.00%,其他组别则难以达到100.00%.而且新型超高频彩超检查组较其他三组影像检查项目在某些方面的优势更加明显,在眼球屈光系统疾病的极早期就能发现.结论 眼球屈光系统的主要成分是水,在物理学上为超声波提供了良好的背景板,使眼球各种组织在超声屏幕上产生了很高的对比度.超声频率越高,对比度、清晰度越好.因此超高频彩超(14~ 20MHz)检查是近年来应用在眼科疾病尤其是眼球屈光系统疾病诊治中重要的检查手段.
