阴道分泌物标本五联检验对阴道炎病原体及白带清洁度的临床诊断价值分析

摘要

Objective To investigate the clinical diagnostic value of vaginal secretion 5-item combined inspection in vaginitis pathogen and leucorrhea cleanliness.Methods A total of 643 women undergoing routine leucorrhea examination were enrolled in the study from May 2016 to May 2017 in the outpatient department of our hospital.Two vaginal secretion samples were taken from each person,for routine microscopic examination and vaginitis 5-item combined inspection respectively.Compared the detection rates of two methods for the pathogens of vaginitis and the cleanliness of leucorrhea.Results The positive detection rate of Gardnerella vaginalis and lactobacillus by vaginitis 5-item combined inspection were 59.7％ and 74.3％,significantly higher than 39.1％ and 56.8％ by routine microscopic examination (P＜0.05),but there were no statistically significant differences in white blood cell,Candida,trichomonas,and pH value between the two methods (P＞0.05).The positive detection rate of bacterial vaginitis and mixed infection by vaginitis 5-item combined inspection were 62.9％ and 30.3％,significantly higher than those by routine microscopic examination (P＜0.05),but there was no statistically significant difference in the overall positive detection rate of vaginitis between the two methods (P＞0.05).In the leucorrhea cleanliness,the positive rate of Ⅲ + Ⅳ degree tested by vaginitis 5-item combined inspection was 56.6％,higher than 51.7％ by routine microscopic examination,but there was no statistically significant difference (P＞0.05).Conclusion Vaginal secretion 5-item combined inspection can increase the detection rate of bacterial vaginitis,obviously reflect the vaginal micro ecological environment,with simple operation,which is worth promoting in clinical practice.%目的 探讨阴道分泌物标本五联检验对阴道炎病原体及白带清洁度的临床诊断价值.方法 选取2016年5月至2017年5月在本院门诊接受白带常规检查的女性作为研究对象,共有643人纳入研究.一人分取两份阴道分泌物样本,分作常规镜检和阴道炎五联检,比较两种方法对于阴道炎病原体及白带清洁度的检测率.结果 阴道炎五联检检测加特纳菌和乳酸杆菌的阳性率为59.7％和74.3％,明显高于常规镜检的39.1％和56.8％ (P＜0.05),而在白细胞、念珠菌、滴虫和pH值上两者差距不大(P＞0.05).阴道炎五联检对细菌性阴道炎和混合感染检测阳性率为62.9％和30.3％,明显高于常规镜检法(P＜ 0.05),但二者整体对阴道炎检测阳性率无明显差异(P＞0.05).在白带清洁度上,阴道炎五联检在Ⅲ度+Ⅳ度的阳性率为56.6％,高于常规镜检的51.7％,但差异不明显(P＞0.05).结论 阴道分泌物标本五联检验可有效提高细菌性阴道炎的检出率,较明显反映阴道微生态环境,操作简单,值得临床推广.
