Objective To investigate clinical features of Rhodococcus equi infection in AIDS patients. Methods The data including clinical manifestations, laboratory test, imaging examination, treatment and prognosis of five AIDS patients complicated by Rhodococcus equi infection, who were admitted to our hospital from Oct 2008 to Sept. 2010, were retrospectively analyzed. Results The five patients aged from 32 to 58 (median 34) years old. They were all in a stage IV of HIV infection at the onset with lower CD4+ T lymphocyte counts, and 3 had other complications simultaneously. The most common symptoms were fever, cough, chest pain and bloody phlegm. Only 1 patient had elevated white blood cell counts. Four patients improved and were discharged from hospital, and 1 gave up treatment. Conclusions The diagnosis of Rhodococcus equi infection in AIDS patients depends on pathogenic examination. Its clinical features are diverse. Applying antibiotic treatment according to drug sensitivity testing at an early stage, and giving highly active antiretroviral therapy in time can improve the prognosis of AIDS complicated by Rhodococcus equi infection.%目的 总结AIDS并发马红球菌病的临床特点.方法 回顾性分析2008年10月-2010年9月我院收治的5例AIDS并发马红球菌病患者的临床表现、实验室结果、影像学检查、治疗及预后等.结果 患者年龄为32～58(中位数34)岁,发病时为HIV感染的Ⅳ期,CD4+T淋巴细胞计数均较低,其中3例同时出现其他并发症.主要症状为发热、咳嗽、胸痛、咳血痰等,实验室检查白细胞计数仅1例升高.4例好转出院,1例放弃治疗.结论 AIDS并发马红球菌病的诊断依靠病原学检查,临床表现多样,早期根据药物敏感性试验进行抗菌治疗,并及时进行高效抗反转录病毒治疗可改善患者预后.
