目的：：了解社区45岁以上糖尿病前期人群2年后自然转归情况、特点及影响因素。方法：统计社区45岁以上糖尿病前期人群1465例,两年后进行复查,采取填写调查问卷、二次糖耐量试验,以及糖化血红蛋白、血脂等测定等方式,了解自然转归情况以及不同转归的人群基线特征,分析影响转归的影响因素。结果：共纳入1465例糖尿病前期人群,其中610人转化为血糖正常人群,731人依旧处于 IGR 水平,124人转化为糖尿病,糖尿病转化率为8.5％。在不同的年龄组,随着年龄增长,糖代谢异常、高血压、肥胖、脂代谢异常均有增加趋势,而血糖升高可能与增龄、收缩压(SBP)增加、血脂代谢异常等密切相关。进行 Logistic 回归分析,高龄、血压升高、血脂代谢异常均为糖尿病危险因素。结论：以是否进展为糖尿病为因变量,进行 logistic 回归分析,发现基线 FPG、2hPG、超重和肥胖是影响糖尿病前期转化为糖尿病的独立危险因素。且转化为糖尿病者男性比例、尿酸、BMI 和收缩压均高于未转为糖尿病者,调查结果具有统计学意义。%Objective: To analyse the natural prognosis, characteristics and influence factors of people over 45 years old with diabetes in the community in two years. Methods: 1465 cases of early diabetic popula-tion aged over 45 in community were chosen as objects. The natural outcome and baseline characteristics of the population with different outcomes and the reasons were analyzed by filling in the questionnaire,two glucose tolerance test,glycosylated hemoglobin and blood lipid measurement after two years. Results:The total of 1465 cases of early diabetic population, which 610 people into a normal blood sugar, 731 people are still in the IGR levels, 124 were converted to diabetes, and the conversion rate was 8.5%. In the different age groups, along with the age growth, abnormal glucose metabolism, hypertension, obesity, abnormal lipid metabolism were in-creased, and hyperglycemia may be associated with increasing age, systolic blood pressure (SBP) increased, blood liquid metabolic abnormalities are closely related.Conclusion: Logistic regression analysis, age, high blood pressure, abnormal blood lipid metabolism are risk factors for diabetes. The male proportion, uric acid, BMI and systolic blood pressure of those translating into diabetes are higher than of those non-translating into diabetes. The survey results are statistically significant.
